- A photocopy of the EPI vaccination card, or a copy of the hospital discharge paper if the child was born in a hospital, or a certificate from a health worker
- Any tax payment receipt or a copy of an electricity bill as proof of address
- Parents’ digital birth registration numbers and photocopies of their National ID cards
- A mobile phone number
After 5 Years of Age
However, if you apply for birth registration after the child is 5 years old, you will need the documents mentioned above.
You will also need a Primary School Completion Certificate or any other educational certificate.
But if these certificates are not available, the vaccination (EPI) card will be enough.

Step 1: Select Address
In the first step of the online birth registration application, you need to provide the applicant’s information.
- First, visit the Birth Registration Application website.
- After visiting the site, tap on the Birth Registration option and then Application for Birth Registration.

- Here, you will see mainly two options: Place of Birth and Permanent Address.
- This means you have to choose from which address you want to collect your birth registration certificate after completing the online application.
- If you want to collect it from the place of birth, select the Place of Birth option.
- If your place of birth and permanent address are the same, you can select any one of them.
- If you want to apply for birth registration from abroad, tick the option that says: “If you want to apply for birth registration at a Bangladesh Embassy, select this option.”
- Then click the Next button.
Step 2: Provide Information of the Person to be Registered
In the second step of the online birth registration application, you have to provide the identity of the person to be registered.

- Write the first and last part of the name in Bangla and English. That means if someone’s name has two or three words, you will write the first one or two words first, and put the remaining word in the last name section.
- After that, you have to provide the date of birth and gender, then the other information.
- Then go to the Birthplace Address section.

- Here you have to provide country, division, district, city corporation or upazila, municipality or union, ward number, post office in Bangla and English, village/area/mohalla in Bangla and English, house and road number.

It is not possible to register a birth without parents’ online birth registration number (17-digit number). Step 4: Provide Permanent and Present Address
In this step, you have to select the permanent and present address.

- First, the permanent address section will appear.
- Above it, you will see an option named “Birthplace address and permanent address are the same.”
- If your birthplace address and permanent address are the same, then you must put a tick mark there and fill in the options below.
- After giving this information, come to the present address section.
- Just above the present address section, you will see an option named “Permanent address and present address are the same.”
- If your permanent address and present address are the same, then put a tick mark on the left side of it. If you tick this option, you do not need to fill in the present address section again.
- After giving all the information, press the Next button.
Step 5: Provide Applicant’s Information and Attachment
In this step, you have to provide the information of the person who is applying for the registration.

- If you want to apply for your own birth registration, then you must select the “Self” option and give your name and phone number below.
- If someone else applies for the birth registration, then select the “Others” option.
- Then provide the applicant’s name and phone number. If there is an email, then give the email on the right side. If there is no email, there is no problem.
- After giving this information, press the Next button to complete the application.
- Here, there will be instructions about what kind of documents you need to upload.
- You only need to upload any two documents.
- After uploading the documents, click the “Submit” button.

- Then, provide the mobile number and an email address.
- After that, verify the mobile number with the given OTP.

- If you do not have a printer, you can download it as a PDF file and later go to any printer service center to print it.

- First, if you register the birth within 45 days of birth, there is no government fee. It is completely free.

- If the child is over 5 years old, the fee for birth registration is 50 taka.
